Before your first visit

Bring any X-rays, scans or treatment plans you already have, along with a list of medications. Previous imaging often shortens the appointment and always sharpens the reading.

Call 201‑447‑9700 and the forms will be sent to you, or complete them at the practice before your appointment. Allow an extra fifteen minutes if you would rather do them on the day.

How early should I arrive?

Fifteen minutes is enough if you are completing the forms at the practice. If you complete them beforehand, arrive at your appointment time.

What should I bring?

Any existing X-rays, scans or treatment plans, and a list of your current medications.
